Catts leaves - whats wrong?

Hi all,

I'd like to here any comments or sugesstions about this troubled catt. I've had it since it was a little tike (about 3 years) I'm not sure what the problem is, I'm not having this problem with my other catts.
I don't know if it could be nutrient deficiency, phosphorus toxicity? Something else .....?

Its potted in a combo of sphagnum moss, bark and course rok.
Ph 7.2 med hard water. Ferts 8-7-6 every 3rd watering.
It seems to start from the top and work its way down, even the most *recent growth* has started, its not from older growth as I though in the beginning. I would think these bulbs would live much longer then 1.6 years.
