I had a Catt that did the same thing , it always produced a leaf out of the sheath instead of a flower . After a few years I tossed it into the trash.

Hmm...I hope that's not going to be the case with her. Some of her growths are indeed bifoliate. She has 8 growths, 3 are bifoliate and 5 are unifoliate.

But this looks to you like it's leaf-growth from the sheath?
