I am guessing that the "no roots" came from being too wet? If you can maintain a mounted plant (it will need daily watering) these grow well mounted - any wood with a rough surface (firmly attached bark), or cork, or tree fern (hapu'u slab). When mounting, you can add a little sphagnum under it for extra moisture-retaining, but don't overdo it - when it grows roots, you want the roots to go into the mount not the sphagnum.

What potting medium you use, or whether to mount it, will depend upon your growing conditions and ability to water it.
Where do you live, and what are typical growing temps?
It's Phal. tetraspis, by the way.

Any air roots? Either way, click on that link that ray has, and get a bottle of kelpmax, it's pretty much steroids for roots

Roberta asked the main question - what led to root death? You need to change something so it doesn't happen again.
People grow this successfully in bark of various sizes, sphagnum moss, mounted on a branch, clay LECA balls, cinders.... What matters is how you water it and the ambient surroundings.
Realize this is a warm-growing Phal. It won't be happy if it goes under 60-65 F / 15-18C.

Mine is growing in a plastic pot with smaller sized bark and s/m. Once I started the humidifier for winter months,it was placed closer to it and I decreased waterings somewhat. Night temps get to ~64 and I think warmer would be beneficial. But a new leaf and roots have appeared. For me it's a slow grower and has been fussy but even at my age I can learn from my mistakes. Good luck!

At what age DON'T we learn from our mistakes? I would hope that it is as long as we love and grow orchids.
