Hi Ajax, the plant looks healthy to me, just ready for a re -pot. Definitely get rid of the moss, bark works well for Phals, at least in my house. Roots that are not exposed to any light will be pale, but can still be healthy. When mine outgrew the small plastic pots, I moved to larger plastic baskets ("net pots"?) in a bark mix. It keeps the humidity up around the core of the roots but lets some roots poke out around the edges, I like the way they look. Don't try to force all those aerial roots back down in the new pot, they'll either keep going or die back if they get too dry.
I only have a few plants so can fuss around with the watering--once a week in winter, two to three times when it's hot and dry here, more if they look droopy. I always give a good flow-through plain water rinse of the whole plant and pot at least once a month to flush out any accumulating salts--water here has a lot of calcium. Other weeks I do a soak with a very small amount of fertilizer, and kelpmax once a month.
Different methods seem to work for different people and different plants. A lot depends on temperature and humidity where you are growing, how many plants you have and how much time you can devote to them.
