The top roots don't look so good. Re-pot and look at the condition of the roots before proceeding.

Also, check for pests on the leaves, mites might be killing the leaf on your first picture.

And I think yout medium should not have coconut husk. Use bark only with leca (or similar).

I actually heard the contrary, as medium is just a way to keep high humidity at the roots, coconut husks are excellent for Phals and used in many nurseries as it is cheap and effective.

However, you should always be careful of overwatering which may be the case for the second one.
