Advice needed for my ailing orchid

Hello, everyone!
I am new to this forum, and I came here to look for help.
I bought a pot of phalaenopsis last year, and it was growing okay with new leaves and nee aerial roots, though its leaves often grew dull and lacklustre after fully grown.

One month ago, I started to give it some organice fertilizing solutions, about once every two weeks, but then its leaves soon started to turn yellow and fall off one after one.
Now there is only one leaf left.

What's wrong with it?
How can I save it?

What's it potted in please? What strength fertiliser were you using?

Are the leaves turning yellow starting from the bottom ones, or from the top?

The old leaves started to turn yellow first. Three leaves fell off already.

It is potted in sphagnum moss . The fertilizer that I used was in powder form (Hyponex), 1g diluted with 1 litre of water and it was poured on the moss.

In you shoes, I would have that sucker out and have a look at the roots.
