I would recommend a clear pot with lots of holes and a mix of sphagnum and medium bark chunks. Remember Phals need to have almost dry media before watering again. It has come a long way and I think it will be fine. Do not over pot it as the media will stay wet for too long.

I will definitely keep an eye on it. I just placed a mix of wood chips, charcoal, and perlite in water and will leave it there for a week so it can absorb water while I keep an eye on the orchid.
Since this is my first orchid that I have saved from dying, every brown part I see scares me and makes me want to remove it before it spreads.
I have been meaning to cut the bottom part but out of fear that it will somehow negatively impact the health of the orchid, it has deterred me away from cutting it.

So it's ok that the roots are loose? It just worries me that they were firm all the way to the orchid's base and they just recently became loose. My first thought is they are rotting since softness is never a good sign.
Thank you, I feel luck has been a great part of saving this orchid.

I will definitely be potting her this week if everything goes ok. I'm going to have her under observation while the potting mix absorbs water.
