Are keikies that are growing from the Phal. pot a good thing or do they spell trouble?

Keiki growth may be because a plant is in distress - but, many Phals (and some other types of orchids) grow keikis as a matter of course. If your plant appears healthy (roots, foliage), it's not something to worry about.

I'm always expecting my orchids to grow keikis but they don't.

Take another picture of that b/c everything but the leaf or what not at the top looks like a spike. However some orchids do produce adventitious leaves on their spikes.

Seems to be a bloom but it is somewhat defocused. Another picture would be great. In my experience with Phal, usually keikis appear in the spikes' nodes.
