Keiki growth may be because a plant is in distress - but, many Phals (and some other types of orchids) grow keikis as a matter of course. If your plant appears healthy (roots, foliage), it's not something to worry about.
Take another picture of that b/c everything but the leaf or what not at the top looks like a spike. However some orchids do produce adventitious leaves on their spikes.
Seems to be a bloom but it is somewhat defocused. Another picture would be great. In my experience with Phal, usually keikis appear in the spikes' nodes.
