My Phal. schilleriana will occasionally grow leaves from the spike nodes and they never turn into keikis.
My schillie was awarded and was happy when I thought I could get a duplicate of my plant but nope, none of the leaves ever grew roots - the spikes eventually dried up and turned brown.

Awesome! It certainly looks to me like this is just a spike that grows adventitious leaves given that it is doing it again. Will be cool in bloom I think. Thanks so much for keeping us updated.
There's new post right now by someone with a new Phal with a spike that has a rather large leaf at the lowest node. I wasn't sure what I was looking at with that one, and forgot all about this thread of yours (sorry!). But how ironic that two different people are here with similar cases although their spike is coming from the base of their plant. So maybe in their case it was a basal keiki that decided it'd rather be a spike but check it out!

I'm on vacation now and my 'chids are home with a sitter. When we left, it looked like it was going to produce a flower at each leaf, but I'll have to let you know when I get back.

I have this same thing going on with my phal. When I purchased my phal. about 15 months ago, it was in a small starter pot. 2 spikes of flowers growing. 1 spike of flower grew from the side of the trunk, the other spike grew straight out of the center top where the leaves form. After both stocks lost their blooms, I went to cut the stocks down and noticed a leaf growing on the stock. So, I kept part of the stock with the leaf. I then transplanted. The stock in the center top. it's probably 3 inches tall and still has it's leaf. The other spike died naturally. Now what I've noticed is a very healthy keiki growing from the main trunk of the phal. it has 2 leaves and plenty of roots growing. The keiki is below all of the other leaves. it's been growing I estimate since I cut the flower stems along with transplant and that was probably 12 months ago.

Someone else will have to chime in here but I do believe that spike coming out of the center is considered a terminal spike which suggests the plant itself is near its end. However I have read here about some people's plants not dying. I have one with a terminal spike for a year now...we'll see what happens. That said, it's possible that is why the plant is now growing a basal keiki.
