This recently happened to me as well on a thriving, well-established phal. It got a new leaf, and when the leaf was about 5.5" long, it started growing a second new leaf. The first leaf remains about 6" long, so it only grew about 0.5" since the second new leaf showed up, and it's somewhat smaller than the other leaves, but not by much. Now the same phal is also growing offshoot spikes from both old spikes, so I wouldn't be surprised if the second new leaf ends up even smaller in size.
