Lost the tag....and it's starting to look sad...

So I have a beautiful new orchid....

A) I have no idea what it is....thoughts?

B) it seems to have a problem....brown, mushy pseudo bulb. Yellowing leaf.

Also, missed a picture of it but a whole pseudo bulb plus baby leaves was beginning to yellow....when I went to look at it it just fell right off in my hand

You have a lovely Miltoniopsis. For the most part it looks fine. But I wonder, is it sitting in water in that cache pot? How are you caring for it?

silken is right, the cache pot must be giving the plant stagnant water and rotting the roots. I suggest you remove it from the plastic cache pot, put it under the sink and let liberal amounts of water wash through the inner pot and make sure the water goes through the holes and wash any debris in the media....then let it drip and let the media dry out before the next watering. Do not put it back in the cache pot until the roots have aerated and the whole plant have adjusted to your environment.

Agree..looks getting overwatered. As much as it likes being moist, it does not need to be swimming in water below. Needs good air circulation too.
