I wonder if it is not necessarily so much of a chlorosis problem as much as a "sun bleaching" type of problem. When I first started growing under lights, some of the older leaves on my phals did get a little bit of yellow bleaching spots but it wasn't as extensive as yours. After moving the light a little further away and gradually increasing the amount of "on" time with the lights, the yellow spots stopped appearing. I think the reason that the new leaf might be okay is because the newer leaf is still growing, which in plant hormone terms means that it can still adapt to any change in environment a lot more than the older leaves that are already "set" can. This might explain your situation.

At this point, it does take up less room and as long as the main crown of the plant and the new leaf/leaves seem healthy, I don't think it's something to worry about too much.

Well, found the culprit. Believe it or not...a single fungus gnat larvae.

When the wound from the fallen leaf didnt dry up and heal, i knew something was up, so I unpotted it. I peeled away the dead petioles of the two recent leaves that had dropped, and found the problem. Physical damage to the collar. I have seen that type of damage before, with an african violet...but i frowned and didnt believe it until i found the culprit. I was so mad I picked up the little maggot and fed it to my fish. So it was hiding in the leaf petioles munching on my poor plant

I cannot believe this is the SECOND TIME an entire plant has been undone by one/two tiny insects. And fungus gnats, of all things.
