Wow that is really cool looking. Can't tell you why it formed that way. Mark that stem and see if it also produces the same double flower next year to.

There will be those who will try to tell you this is caused by hybridizing or over-cloning or some such nonsense. It just happens sometimes. It might be some kind of stress to the plant at a sensitive stage of bud development, or it might not. It probably isn't anything you are doing wrong. It probably won't happen again unless other buds yet to open were affected at the same time.

Yeah that is pretty cool. Perhaps a mutation? I have a mini Phal whose stem was one little big conglomerate. The branches of the spike never separated from the main stem thus many of the buds were all smooshed together at the top/end. It actually is still in bloom though most of the buds wound up blasting and those blooms that survived never fully opened. I still like it

Cool plant though and thanks for sharing!

It happens sometimes ...
Over use of hormone product may contribute to it.

you can call them fused but they more confused to me ya know ..

I have that happen more to me on the catts that the petals will get fused together like that from 2 flowers or a single one and not grow properly into bloom. You can tell when its in bud that the flower is going to be wonky. I let them be doesnt hurt to look now thats it flowered.

For the same reason human sometimes produce siamesic twins: it just happens. Something genetic goes wrong in an early stage, I think its just something that happens- but not very often- usually things doesnt survive when nature screw up, sometimes it does. I like the odd things- beautiful in all its craziness whats interesting is to see if it does it again, like if it is in the dna or if its a random "once in a lifetime" thing.

I have no idea and no scientific reasoning behind this- just my thoughts, so dont take it to serious
