This sounds like your Den might have been a cane propagation, that p-bulb was could be a cutting from the parent.

Oh, that's interesting.. I would never have thought that since the plant has a few unusually short pseudobulbs and one very, very small leafless one. Then again, I dunno nothin' bout anythin' concerning propagation.
