Are you certain that's a 3" - 4" pot?

That's looking quite big in your hands. It looks like a 5" or 6" pot. I just looked at some orchids potted in what I know are 6" pots, and my hand looked just as obscured as your hand was in that pic.

If it was a 3" pot, I would be able to see more of your hand in that photo.

There's a good amount of good roots, I wouldn't cover the plant.

Why do you feel the plant is dying?

Ok your right, I just measured the pot it's 5" diameter x 5" tall. The roots touched the edges though. I was gonna say its an optical illusion cause I have small hands. Haha

Could moving to a house with a water filtration system change the amount of water they're consuming or cause problems? Maybe the move shocked them? Idk, I've had this particular plant for 5+ years. I believe I've revived it once before too.

I would just cut off the rotted roots repot in bark and moss and it should be good to go. Heck I've saved worse looking ones! Good luck!

It likely has declined, but seems to be in fairly decent shape. Cutting away rotted roots, repotting into appropriate media and smaller pot, along with proper care is all it needs, imo.
