There are 2 hybrid Phalaenopsis that I see commonly sold that look like yours. One is called P. Dark Knight and the other is P. Elegant Deborah (I have the latter one). The name of the hybrid is probably laser-printed on the side of the pot.

I agree with others, this plant is far from dead. It just needs more consistent water and care. Lots of good advice given above.

Indirectly you helped me to find the name of my hybrid phal...
I think that the name "Cambridge" printed on the pot was only an information about the pot vendor, but the pic i just found is equal to my orchid

P.S.: the attachment is the Cambridge phal hybrid i've just discovered
