I think these have been getting too much water. I gave my daughter a phal and she rarely remembers to water it and it is thriving. Phals grow on trees, roots exposed to the humid air. The medium in which you plant them is only meant to help keep the air around the roots humid, not to keep them wet. When the medium stays damp, the roots rot.
You might try wrapping a little bit of slightly damp, fresh New Zealand sphagnum moss around the rootless step and sealing it in a clear, plastic bag, low light (it needs some light, of course). Good luck!
