I have this orchid plant which has a similar flower form with your orchid:

Lepanopsis Michelle
(Lepanthopsis floripectin x Lepanthes cochleariformis)
This plant came from Lynn O'Shaughnessy.

This is a very old heirloom orchid plant dates back in the early 70's way back before I was born, I inherited it from a guy who was returning to Europe 3 years ago....I know it came from Lynn O'Shaughnessy because the other side of the tag has her name, address and phone number printed on it.
This spring when I repot I will give you four leaf forms to start your own cluster. Its a promise.

Correction:

this hybrid plant was made by Moises Behar, MD, in April 1991, sown in August and protocorms were reflasked in November and finally started to flower in the flask in January 1993.

sure Carrie....as of now I have 28 leaf forms....and these plants are not like Cattleya bulbs that needs at least 3 backbulbs to pot.

That is so true, I had one tiny little division that broke off the mother plant, maybe some 3 leaves, placed it on the top of Bulbo pot on sphagnum and it is growing:-)
