Yep, I agree that it looks like an old leaf. I agree with James suggestion, but I just wanna add that you don't want any of your Phal's roots touching the water in the bottom of the jar... That could lead to rot, so just make sure the water level is below any & all roots.

Thanks Mary Linn. I forgot to add that part. Also when you take off the moss, do it S...L...O...W...L...Y. Start at the bottom and work up and out. Carefully with tweezers.
