Cheryl, what kind of temp range is it experiencing? ranwild site says that initiation of flowering shoot is promoted by 18-20C night and 25-27C day for a month. Some of my young phals (all species) make flowering shoots in one season, but wait for the next season to flower.
Somehow, it is not happy if you are seeing 2 new leaves in 2 years. In Ontario, can you really keep 60% minimum RH in winter? It sounds difficult without humidification (or enclosure). If you can keep 60%, it should be ok, but the leaves look a bit dry-ish (dosen't have the luster of phal leaves, and the older leaves shows raised veins).
I was just reading a blog where the writer said as long as you provide what they want, they will not do anything for you. In other words, provide fertilizer, they get "lazy" and do not grow roots. Today I saw a post where the leaves were getting smaller and not larger because they were reacting to getting too much sunlight. Similar thing.
I personally think there may be a problem with your fertilizer numbers. During rest period (I have read) the numbers need to be the same like 10-10-10, but during spiking/blooming it needs to be second number biggest like 10-30-10. Could this be true?
