Yes! Clean it up a little (cut the dead spike closer to the base, remove dead material). Spray the bottom with half and half (hydrogen peroxide/water)--it has a little fungus according to pic#3. Good luck! :P

Yes, though could take time and it's not for certain.

I would pot it up in a tiny pot, the smallest you can fit the roots in, with your medium of choice. Then water often but also let it dry between waterings. If it's not drying fast enough to water frequently and also let it dry out then cut slits in the pot or use a faster drying medium. Rescues seem to like frequent watering but also drying.

I would also use luke warm water when watering, and possibly a seaweed supliment to try and promote root growth.

I have a few rescues that I am trying the "sphag and bag" method with and so far they are hanging on. One was pretty similar condition to yours. It's not certain that it will make it but it looks like it is doing ok now.

Thank you all,
read all 61 pages of phal abuse, cleaned it up, and potted it in the smallest pot I could find, the bottom quarter of a 1 pint water bottle.
