Spike nubbins appear at the 'crotch' of a leaf. BUT sometimes roots appear there as well, so it can be confusing. Usually each 'crotch' only makes ones spike (though I've had cases were I snapped a spike and a new one came from the same spot), and in general the next spike appears one leaf further up from the previous spike (so on the next leaf, on the other side of the plant). I have one Phal that spikes quite randomly though, and will bloom sometimes bloom 2 or 3 leaves up from the previous spike.
Anything white is a root for sure.
