Also, james mickelso is correct in what he said. So it's really your choice.

As for fertilising, a normal balanced fertiliser is fine. I use 20-20-20, very diluted.
Same here. I've got two keikis that have formed from me not cutting the stem. I just wait and let the orchid decide if it wants to be done using it and let it dry up before I cut it.
