Any pictures of a new flower spike? I think I have one on my Phal which looks healthy. There is a shoot coming out from the crown and I was wondering if I need to train it vertically or does that happen naturally?
If it really is a spike coming out of the crown, then that's not good, it's known as a terminal spike. Meaning that the plant will be incapable of growing new leaves. Do you have a photo?

Regarding cutting or no cutting?
I personally do not cut any spike as long as they stay green. As for phals, some of mine keep green long after all the flowers are gone. I just leave them as it is. Others of mine will turn brown from the tip and gradually dry out all the way to the base where it came out of. Then I cut it off.
I like to let the plants do what it likes to do.
