Salt build up

Hi all, I have read that some orchids get a salt build up that will harm the plant. Where does this salt come from? How do you know when it is building up? How do you remove the salt? I water regular with rain water (if they look thristy they get a drink) and fertilize once a month with Rays orchid fertilizer/distilled water. Thanks for the help!
