Sorry to say this, but it's too far gone.
If you decide to try again, there's lots of info on the OB regarding Phalaenopsis culture. The easiest place to start is the sticky called "The Phal abuse ends here."

It looks like you have crown rot and possibly root rot.
I'm sorry but I agree with King, this plant is too far gone to save.

Yup, too far gone. I'd vote like Joann did, with a side of too much water. Sorry, it happens. Better luck with your next one!

I agree with the others. Crown rot and likely root rot. The media looks pretty wet in the picture. Toss it and treat yourself to a new one. But make sure the new one isn't potted in soaking wet moss packed tight in the centre of the root ball. That's the reason many phals end up in orchid heaven.

I found a homade fungicide/insectside that I am treating the orchid with!!! Mabye I am not giving the orchid enough light & the right kind of fertilizer!! I am wondering if i need to buy a grow light for it!!! Physan will probably help treat it too!!!

Phals like indirect light and do OK near a window so long as there isn't direct bright sun. So a light is likely not necessary. It just looks like the roots and crown (where the leaves come from in the centre) got wet and are rotting. Physan and other fungicides help these problems but sometimes things are too far gone and they don't work miracles. I wouldn't feed it at this time as it likely can't take up many nutrients and you don't want to burn any root that is left. Go ahead and try a bit of fungicide, nothing to lose, but it is pretty much ready for orchid heaven. Good luck.

A plastic bag over the orchid at this juncture is not going to help much in my opinion, and in fact may exacerbate the problem.
Literally adhering to the sphag and bag technique is in my opinion a pretty hit or miss method. I'd probably even venture to say that it leans more towards miss than hit.
The problem is that there is not enough gas exchange happening in the bag, and it's wet in there, which may cause bacteria or fungi to proliferate like mad.

If it makes you feel any better...
A typical NOID Phal hybrid is mass produced by the thousands. One less NOID Phal hybrid isn't going to cause problems.
Species Phals are a different story entirely.
I think it'd be better if you got another one. I think you'd be much happier.
At least now you know where to go if you needed info or help.
