"Keiki" is when a new cane begins to grow above the surface of the potting medium (like, sprouting out of a parent cane halfway up in the air). This little new cane will get bigger and eventually grow a bunch of new aerial roots of its own. At that point it could be removed, with its own roots, from the "mother" and potted on its own as a new plant.
The only way a keiki is different from the new canes growing from the base is that its roots are completely in the air.
Keikis can also simply be left on the mother. The mother gets bigger faster and can turn into a very impressive plant with many many flower spikes if its keikis mature on it without being removed.
