You should tell about your growing conditions, outside greenhouse, window, under lights, watering freq, temps subjected to.. it may clue someone in to the problem!

It's growing on a plant next to an east facing window being watered once or twice a week allowed to sit in about 2 cm. of water with a fogger on once a day for 30 min to an hour depending on the temp. The day time temperture is around 75 degrees and a night time temp. of around 65 degrees

Is it growing on a plant or plant stand? How and what are you fertilizing with?

Actually, I think the problem is that you really do not feed your plant... (would you be happy having just water and no food for two years?).. Paph villosum tolerates pretty well cool/cold temperatures down to 8°C would not kill it nor affect it very much! (Acc. to Sliperorchids, "Mean Temperature Range: 8-19°C")

Furthermore, you said that your plant is allowed to sit in about 2 cm of water... what kind of medium is the plant potted in? is it still ok? how are the roots of your plant?

Showing a photo of the plant would help to see if there is any other problem on that plant!
