I see a couple issues. First, the original pseudobulb pleating was not your problem. It came that way. I see no pleating on the new pseudobulb.
Second. It's very common to purchase Oncidium class orchids with only one pseudobulb, in blossom, and in poor shape over-all. I have personally nursed a rootless pseudobulb back to a blooming condition.
My preferense (and I have lots of these guys) is to grow them in an open bark mix where I can water then twice a week or more and the roots won't rot.
