Default 3 new possiable spikes and something weird!

I got 4 plants, 3 appear to be putting out new spikes, cool beans, but this 4th, had the same green nipple thing, its been there longer than the others, but it popped open and it looks like its going to be a leaf, what in the world?

Not to be a party pooper, but I think that the first 3 are new roots.

I'd say 1 and 3 are probably roots, 2 might be a spike and the last two are just plain weird!

dang roots, I thought I was going to have more flowers and about the keiki, I know very little about those, but I thought keikis grew on old spikes? So what do I do with it?

Some phals are more prone than others to form basal kiekis. I have also seen a few, very few that decided to branch similar to what vandas do. If you want to remove and have a sencond plant wait until it has a few roots of it own. By the way, phals with multiple crowns are beautiful in bloom.

So if I left that keiki on there, it will grow in to a normal size plant? and have flowers to boot, sounds cool to me.

Yes, twice the spikes in one pot, eventually. And I agree with others, 1 and 3 definitely roots, probably 2 too.
