Default Black hole on Phal crown.

So here it is, I checked it out closely and it does not appear to be rot, the rest of the plant is in great condition, so I am sure I am dealing with something new for me and just about anything will be new to me

Is this one of the Bill Asia phals? Do you have a picture of the full plant?
My It looks like the new leaf rotted, possibly from trapped water. Do I see webs and spots on the leaves? (sometimes the color on my monitor isn't the best)

I thought it looked like rot too. Not sure if that's webbing - looks like it could be mineral accumulation

No!! I have no bill asia plants as of yet!! actually this is my very 1st Phal I got back around Febuary, those white fibers you see are more than likely paper towel fibers as anytime water gets on the crown, I wipe it dry right away, the area thats black is hard, unlike what i have heard rot thats mushy and if you notice further down the crown below the black hole, its still kind of green and firm.

Glad to know it's paper towel residue instead of the dreaded mealies. I've not seen "hard" rot and it doesn't look like mechanical damage so I'm clueless.
Hopefully a new leaf will grow soon and there will be no further problems. Looks like you're going to have a lovely display of flowers soon.

I am quite certain its not rot, in fact now that I think about it, I seen a black spot on that leaf before, but there was no hole, it was just on the surface and I didn't think much of it, until that leaf started dying, hopefully someone will see this that might have dealt with this before, at this point I do not know what to do about it.

Yes! there is going to be some nice flowers, small, but nice
