Question Den. phalaenopsis has damage but from what

I have had this orchid for a year. In the spring I began to see whitish/brownish "scabs" on a few of the canes. In addition the leaves have become wrinkled and are bending in different directions. Some are floppy and beginning to turn yellow. Also some are a dark, dull green color. It has a new growth and new roots, but I fear the new growth will develop in the same way as the other new growths.

What do you think it is and what are my next steps?

I would try more water and light but I only have a couple Den's so I'm no expert. Give some more details on your culture and you will get some better advice most likely.

Thanks for your response cabbo. Now that it is warmer I will water more frequently. Hopefully that will help with the drooping leaves.

To add a little more detail,

1. I use several 26W CFL lights which makes 2500 fc.
2. My humidity is usually 35-45% in winter and 60-70% in summer.
3. I never take my orchids outdoors.
4. I water once a week and fertilize "weekly, weakly" for three weeks and flush with water on the fourth week.
5. It was originally mounted on coconut husk/shell and then repotted into fir bark.

At first I thought it was due to the type of fertilizer I was using. I read that mineral deficiencies can cause some of the problems I have so I purchased a different kind of fertilizer. I also decided to repot since the coconut husk/shell stayed too moist to the point that it became sticky and molded in spots. I don't know if that will solve the problem so I am hoping someone more experienced may have a solution.
