I have only bought supplies from http://www.calwesttropical.com/index.php and just the one time, but I was very happy!
I am sure others will chime in with more

Yvan mentioned three of the places that will get me in the most trouble on-line for purchasing orchids. For supplies, I love repotme.com. Their imperial mixes are awesome, but you must keep in mind that some potting mediums will hold less water and some more water, you must figure out what is best for your growing area and watering habits.

Orchid Limited is located in Plymouth, Minnesota too in case you haven't stop by. They have nice plants with superb quality, but you have to pay for a higher price. Very big collection of orchids of many genera. Orchids and Orchid Supplies

I now buy (weather permitting) most of my orchids online, many of them on ebay. I especially buy from ebay sellers that show not only a picture of the flowers but a picture of the actual plant I will be getting. I also look for a detailed description. I've been more than pleased with those purchases. On the other hand, I've bought from major orchid companies and been disappointed with some plants and thrilled with others within the same order.
As far as supplies go, I usually look locally first except for items that I can get for less (including shipping) or things I can't find so easily. If there is something I need and can find it online only, I may order other items at the same time because the shipping cost increase is minimal for most additional items.

oak hill garden is great, orchids by hausermanns is too, and there is always carmela in hawaii. supplies are best from lowes or home depot and if necessary oak hill garden and hausermanns both have odds and ends. ebay plants are sometimes good, if you are careful who you buy from. you just have to get to know the vendors i reckon. good luck!

For Just pure supplies I'd recommend-
Calwest Orchid Supplies CA
Green Barn Orchid Supplies FL
I also get some supplies from ebay as well...
For orchids there endless places to get them- word of advice, buy from places that people recommend highly and make sure that the customer satisfaction is their priority!
Places I would recommend for buying:
OrchidTn on ebay
Hausermann
Parkside Orchids
Natts Orchids for Vandas
Odoms for Cattleyas
