This will go against all pot-size convention, but when I move phals into S/H culture, I try to move them into pots that are almost the diameter of the plant's leaf span.

I find that when I do that, the aerial roots redirect themselves down into the medium and anchor the plant really sturdily.

I would repot, that moss looks done. If you do decide to put back inside moss you can add 30 to 50% of perlite to the mix. This will allow good moisture with lots of air.
(IMHO) It's not the moss that's has the problem, it's how you water.

Don't over pot, make sure you repot within 9 months. Fanning is always a good choice this help dry out any media you choose to grow in.

I also had some problems going from moss to a bark mixture - this is actually how I learned to grow in sphag moss. If catts & dens can grow well in sphagnum moss, then ... it's a great choice for phals too.

You will get so many opinions on potting media, everyone has their favorite.
