Default struggling P. (or E.) cochleata

I've been having a hard time with this plant. Most of the roots have died. We just moved into a different house so I'm hopeful the different environmental conditions will suit it.
As you can see it is developing some new very small roots. The substrate is orchid bark with just a little leca thrown in (in the hopes of increasing drainage).
I welcome advice. I would love to see this plant bounce back and thrive.
I've attached photos of the sorry state of the roots and plant and the medium.
