I usually leave them on till the roots get a couple of inches...as you just want the plant to get stronger!

The "standard" answer is that keikis should not be removed until the roots are 2-3 inches long...
that being said...there are no "rules". You do what works for you, right?

What happens if I don't remove it, as I am inclined to. Will it fall off? Will the spike that connects it to the mother plant rot away and separate the two? Or will I have, as time goes on, two adult sized plants connected to each other for ever?

sounds like you wish to remove it...then maybe wait a couple more months and then do so. It will stay on the plant for years before getting too heavy to fall off.

eventually, an equestris can make a great specimen plant, with keikeis and a large mother plant,. and all in flower.
As for the mother plant not flowering, how is your light? Remember, the human eye is deceptive. What looks sufficiently bright, may in fact not even be half what a plant needs.

Frankly, I put this one in a dark corner. The reason is because I decided to try to attach it to my cornplant tree,
