Once or twice a year a couple the leaves on one of my Phals just start to wrinkle and falls off and the plants die. The rest of my plants seem to be healthy. I have looked at the roots and the soil, and they all appear to be normal. I do not see a problem with any type of bugs. Does anyone have an answer to this problem? I have had some of my plants for several years.
If there are no obvious root issues, it could be normal leaf senescence. A phal will get "taller" over time, adding leaves at the top and losing them at the bottom.
A caveat to that is that the better a plant is grown, the more pairs of leaves it will typically carry.

Thanks for the information, but this is not a case of old growth, new growth. All of the leaves (one at a time) start to wrinkle and fall off and at some point, there is no top growth of any kind. I wish that I had a picture, but I do not have one that is doing this right now.
Are you watering your plant from overhead?
It sounds like crown rot to me.
Pull those newer leaves back and let us know what you see.
You could try pouring a few caps of peroxide down in the crown area - give it 10 mins or so, then put the plant on its side and help wick it out.
