you can also do a 'vase culture' successfully....just search on the past posted threads of the senior members....the sphag and bag are for the nearly dying plants and quite a drastic method for this plant....your plant still have healthy leaves but severely rotted roots so you better let it breathe standing over a vase or a jar half filled with water and allow the roots to grow

Honestly, it doesn't look that bad to me. I'd pot it up normally in a clay pot with fresh spag. Just be sure to stake it really well so it absolutely cannot move or wiggle at all.

And the top down shots make it look like the leaves are getting a bit too much light. I can't tell if it is the flash used in the pic or if the leaves are a light green/yellow color.

I see mechanical damage on the leaves but it wont hurt if you sprayed it with PHYSAN 20= is a broad range disinfectant, fungicide, virucide, and algaecide which effectively controls a wide variety of pathogens just to make sure ....
and dispose of the decayed media mix....it looks nasty on that white plate....the roots rotted maybe because the media have already decayed and it was due for a fresh repot= which you can do after you see new root growth on the vase culture

*Steve has a point....if you can repot on a fresh media then it will survive

find a jar or a vase with the opening just right to fit the roots to let it hang inside the container(wire the base of the plant and wire it around the mouth of the glass container so not to let it fall inside the glass and fill the glass with water just to a level that the roots are not touching the water....
you need to spray the roots every morning for humidity and moisture....
once a week fill it up with water and fertilizer to soak it for 30 minutes and then pour it out and fill it with the regular water on a level not touching the roots....

*when you see the plant growing new roots its time to repot it in a media mix....but as Steve said you may repot it now on a fresh media using a smaller pot than the original size it was potted in and skip the vase culture....its up to you

vase culture is great because you can see when the new roots grow.
